Application and Boarding /Grooming Agreement
The following represents a contract between Patricia and John Groover Owners of ShowMyDog, LLC and the Owner of the pet whose name(s) appears on this Application and Facility Agreement (the "Agreement"):
By submitting this Agreement and leaving his/her pet with ShowMyDog, LLC, Owner certifies to the accuracy of all information given in the Agreement. Owner agrees to pay the rate for boarding, grooming and any other requested services in effect on the date the pet arrives.
We are pleased to administer any medications your pet may be required to take. Owner will inform us of any medical problems and or allergies that may flare up due to the stress of grooming at the beginning of each appointment. The owners of ShowMyDog will not be responsible for clipper burn and/or minor nicks resulting from grooming of matted, neglected coats or for irritation caused by removing the coat from pets possessing mild to severe skin allergy. We reserve the right to charge additional fees for services we consider over and above the norm covered by our standard rates. These additional services would include medicated baths, flea baths that could be a potential threat to us or to other pets left in our care, whether it is an aggression problem, health problem or parasite problem.
I understand the conditions for boarding my pet in my home and by signing this release and waiver I am agreeing to hold the owners of ShowMyDog, LLC harmless from any and all claims for damages or injuries suffered by, or in any way relating to my pet's stay with us. The Owner agrees to be solely responsible for any and all acts of behaviour of the pet while in our care. The owners of ShowMyDog, LLC will not be held responsible of stressful effects grooming or boarding may have upon an elderly pet.
All pets must be in good health. Owners will need to certify that their pet(s) are in good health and have not been ill in the past 30 days. On admission all pets must be free from any condition, which could potentially jeopardize other guests and our dogs' health. Pets that have been ill with a communicable condition in the last 30 days will require a veterinarian certification of health to stay with us.
While we do our very best to care for your pet, we cannot guarantee their health during their stay. Some conditions are unavoidable while boarding your pet such as but not limited to weight loss, hair loss or diarrhea. I understand that all pets admitted must be protected against communicable contagious diseases and must be free of parasites. This will help to provide for a healthier environment for all.
